doc fix to make Test::Plan and Test::More syntax equivalent (and tests to make sure it won't happen again) thanks to Thomas Shinnick for the spot
don't pass a skip reason to skip_all if one wasn't provided

To install Test::Plan, copy and paste the appropriate command in to your terminal.

cpanm Test::Plan

perl -MCPAN -e shell
install Test::Plan
